Adult foster care
Care that feels like family
When someone you love needs a little more help, they deserve more than a room down a hallway. With Community Efforts, they live with a caring, trained family in a real home, with personal attention every single day.
What is family care?
It is a simple idea with a big heart. Your loved one moves into the home of a caring family we have carefully chosen and trained. They have their own private bedroom, enjoy family-style, home-cooked meals, and are part of everyday life: holidays, birthdays, and quiet evenings on the porch.
Each home welcomes every individual as part of the family and community. Care is individualized and person-centered, and it promotes independence: helping your loved one keep, build, and grow the everyday skills that let them thrive in a family and community setting. A consistent caregiver who bonds and relates with your loved one is there day after day, so trust grows and nothing gets missed.
Life does not stop at the front door. We encourage community life, with the freedom to come and go, help getting to church, errands, and outings, and rides to doctor visits with a caregiver who attends and follows up. Behind every family, the Community Efforts team visits the home, checks in regularly, and keeps you and the doctors connected. Our goal is simple: care for your loved one as if they were a member of our own family.
A great fit for adults who
- Need daily help, but not a hospital or nursing home
- Would rather live in a home than a facility
- Enjoy company, conversation, and getting out
- Are older adults or adults with physical disabilities
How family care compares
| Family care home | Assisted living | Hourly home care | |
|---|---|---|---|
| Where they live | A private bedroom in a real family home | A room or apartment in a large building | Their own home |
| Who helps them | A consistent caregiver who bonds and relates with them | Staff who change with each shift | Aides who come for scheduled hours |
| Meals | Family-style, home-cooked meals | A dining hall | Whatever is made during a visit |
| Company | A family and a few housemates | Many residents | Often alone between visits |

Peace of mind
Our team visits the home, checks in regularly, and keeps you, the doctors, and the care team connected.

We also work with TennCare CHOICES.
Care coordinators: we welcome your referrals. Please call first rather than emailing medical records, and we will set up a secure way to share them.
